To properly file an appeal, the following information will be needed and staff will assist you:

1. Federal Tax ID Number.
   
   
2. DOR Form 82520, State of Arizona Business Personal Property Statement (filed with the Assessor).
   
   
3. Most recent Personal Property Tax Bill.
   
   
4. Depreciation Schedule from Federal Income Tax Return.
   
   
5. A detailed listing of assets by year of acquisition and a Schedule of Accounts.
   
   
6. Any worksheets or documents supporting the data filed to the Assessor.
